Instructions: Fill out all appropriate sections of this form and
press the SUBMIT button to send your form as EMAIL to the OU Police Dept.
NOTE: While you can use this general form to report ANY hazard to OUPD, it will speed up service if you report certain types of problems DIRECTLY to the appropriate campus office:
For key problems, electrical problems, and lighting problems (including exit, emergency lights, building-exterior and street, or general lighting) —except DARK AREAS (where new lighting is needed) use the OU Facilities Management Report-A-Problem Form.
