Applications

To complete an adult application, follow these steps:

Check/Money Order

One (1) Check or Money Order per Application

*** Please Note: Cash is not accepted

Proof of Citizenship

Original or certified copy of Proof of Citizenship:

State Birth Certificate
Naturalization Certificate
Expired Passport

Primary I.D.

Primary I.D. includes:

Driver’s License
Valid Passport
Military I.D.

*** Please note: if your Driver’s License is issued within less than a year of your application date or from out of the state of Oklahoma, you will need a secondary form of identification with your first and last name pre-printed.

Current Processing Times

Routine 4-6 weeks plus mail time

Expedited 2-3 weeks (additional fee) plus mail time

Items required for Passport Application:

Original or certified copy of birth certificate or previous passport, driver license, and 2nd ID: student ID, work ID etc...
DS-11 Application or DS-82 Renewal Application
Check or Money Order to the U.S. Department of State

For Minors under 16, both parents must be present. For 16 and 17, one parent must be present.

Payments

Application fees must be made by personal check, cashier’s check or money order, made payable to the U.S. Department of State (one check per application)

Any application may be expedited for an additional $60.00 to the U.S. Department of State.

The execution fees and photo fees may be paid by credit card.
