W-2 & 1095-C Tax Forms

W-2 and 1095-C Forms

Each year, employees receive two important tax documents:

  • Form W-2: Reports your taxable income and the amount of federal, state, and other payroll taxes withheld during the year. Forms for the previous tax year are issued each calendar year by the IRS deadline of January 31.
  • Form 1095-C: Provides information about the health insurance coverage offered to you by your employer, as required by the Affordable Care Act. Forms for the previous tax year are issued each calendar year by the IRS deadline of March 2.

How to Access Your Forms

Forms will be mailed to the mailing address on file with the University unless you enroll to receive them electronically.

My Tax Form (W-2 and 1095-C)

  •  Register or log in using your personal information as reflected in Employee Self Service
  • Employees can begin registering 3-5 business days following their first paycheck
  • You can view the MyTaxForm Registration Instructions document for more information

Employee Self-Service (W-2 Only)

  • Click on the Payroll tile
  • Then select the W-2/W-2c Forms tile

Enrolling for electronic delivery offers several benefits:

  • Faster access to your forms—no waiting for mail delivery
  • Secure and convenient online retrieval anytime
  • Reduced risk of lost or delayed mail
  • Environmentally friendly—less paper waste

 


Update Your Information

If you experience any issues while registering please make sure that your information is up-to-date in Employee Self-Service:

If you need to update your personal information such as mailing address or phone number you can do so via Self-Service under: Main Menu > Self-Service > Personal Information > Home and Mailing Address.

My Tax Form - Registration Instructions

In order to view your W-2 or 1095-C online, you will first need to visit the My Tax Form website and register as a user. You can do so by visiting mytaxform.com and using the instructions included below to complete your registration.

Please note: when registering online you will need to use the phone number that is designated as your "home" phone number in PeopleSoft, to check this please log into your Self-Service and select: Main Menu > Self-Service > Personal Information > Phone Numbers. If you are a former employee and no longer have access to PeopleSoft please call Payroll Services for assistance if you are unable to sucessfully register.

If you experience any issues while registering, please make sure that your information is current in Employee Self Service. You can update your personal information (such as mailing address or phone number) by clicking the Update Name & Contact tile.
