Student Forms

• Beginning Fall 2024, a FERPA Release will be required for each Financial Needs Analysis (FNA) submission.
• Students can submit their FNA and FERPA release electronically through the links below.
• Students can visit the Student Financial Center to submit a release and FNA in person.
• All electronic forms must be completed using a student’s verified OU email address.  
• Please read the descriptions of each form carefully.  
• The following forms do not authorize the release of your billing information.
• Students can add a bursar/billing authorized user in One under the Financial tab. 

• FNA - Financial Needs Analysis FERPA Release - No Federal Tax Information (FTI) included
• Complete this form and attach your FNA to authorize the release of your financial aid information, excluding FTI, to an outside scholarship funding source.

• FNA – Financial Needs Analysis FERPA Release including Federal Tax Information (FTI)
• Complete this form and attach your FNA to authorize the release of your financial aid information, including any required FTI, to an outside scholarship funding source.  If your FNA does not require FTI, please use the form listed above.

• FERPA Release – No Federal Tax Information (FTI) included
• Complete this form to authorize the Student Financial Center to discuss your financial aid information, not including FTI, with a third party or parent not included on your FAFSA.

• Federal Tax Information (FTI) Release
• Complete this form to authorize the Student Financial Center to discuss your federal tax information with a parent or third party for the purpose of obtaining financial aid. This form is required to discuss your FTI with a parent contributor listed on the FAFSA.
