Nomination Instructions for Faculty Awards

Faculty nominations may derive from any individual faculty, department chairs or unit directors, elected members of department/unit Committee A, a department/unit's approved nomination committee, center or institute leaders, or a group of faculty who are familiar with the nominee's qualifications for the award. No self-nominations are allowed. With the exception of A2, each of these faculty awards is a one-time award; former winners are not eligible for re-nomination. This does not preclude past winners from being nominated for different award categories. The nominating individual or entity will be responsible for assembling the nomination packets, including supporting documentation. No nomination is required for the A2 award.

Nominations should be submitted electronically as a single PDF file via this online submission form. The faculty nomination packet should include in the following order:

  • A concise summary statement, not more than two pages in length, that describes the impact and significance of the nominee’s research or creative activity justifying nomination for the Award.
  • The nominee's curriculum vitae.
  • One letter of support, preferably from an external source, describing the impact and significance of the nominee’s contribution.
  • A brief statement of the letter writer’s expertise and relationship, if any, to the nominee.
  • Any additional supporting information that the nominator considers to be significant.
