College of Fine Arts Minors
Art MinorStudents must declare this minor in the Fine Arts Dean's office. To enroll in an Art class, the student must contact the instructor of the course and request permission. This form is to be completed with an Art faculty advisor as the student progresses through their Art classes. It must be signed by their Art faculty advisor and submitted to the Fine Arts Dean's Office when they are enrolled in their final course for the minor. Art majors may not complete this minor.
Art History MinorStudents must declare this minor in the Fine Arts Dean's office. Some Art History classes will require instructor permission prior to enrollment. This form is to be completed with an Art History faculty advisor as the student progresses through their Art History classes. It must be signed by their Art History faculty advisor and submitted to the Fine Arts Dean's Office when they are enrolled in their final course for the minor. Art History majors may not complete this minor.
Dance History MinorStudents must declare this minor in the Fine Arts Dean's office. To enroll in a Dance class, the student must contact the School of Dance and request permission. This form is to be completed with a Dance faculty advisor as the student progresses through their Dance classes. It must be signed by their Dance faculty advisor and submitted to the Fine Arts Dean's Office when they are enrolled in their final course for the minor. Dance majors may not complete this minor.
Music MinorStudents must audition and be accepted as a music minor by the School of Music before it may be declared in the Fine Arts Dean's Office. Music majors may not complete this minor.

BME - Teacher Prep Admission FormThis form is required for Bachelor of Music Education majors for admission to the College of Education.
College of Fine Arts Change or Addition of Degree or Minor FormThis form must be completed and then submitted to the College of Fine Arts Dean's Office in Fred Jones Center, Suite 122 for approval.
Concurrent Enrollment RequestThis form is required for undergraduate students pursuing more than one degree.
Course Time Conflict FormThis form is for undergraduate students needing to enroll in classes with a time conflict.
Internship Credit FormThis form is required for students completing an internship for undergraduate credit in College of Fine Arts.
Permission Request for Center for Independent and Distance Learning CourseThis form must be completed and submitted to the Fine Arts Dean's Office in FJC 122 for permission to enroll in a course through the Center for Independent and Distance Learning.

College of Fine Arts Petition to Add a Course after the DeadlineThis form is required to add a class to your schedule after the official enrollment deadline. Students must also attach an "Add Form" signed by the instructor of the class. The Add Form is available in each School's main office or in the Fred Jones Art Center, room 122.
University Petition to Drop a Course After the DeadlineThis form is required to drop a class from your schedule after the official enrollment deadline. Students must also attach a "Drop Form" signed by the instructor of the class. The Drop Form is available in each School's main office or in the Fred Jones Art Center, room 122.
