New Patient Information

Please bring the following with you on your first visit:

  • Social Security Number
  • Insurance Information
    If you have an insurance card, bring it. To bill your insurance carrier(s) for services rendered, we will need the name of the carrier (company), their complete mailing address, your policy's group number and your personal identification number. If your insurance carrier requires a referral form, please bring it with you to your first appointment. If you are covered by Medicare or Medicaid, please bring the appropriate card as well. You may also be asked to pay for services not covered by insurance. Please bring cash, check or major credit card (Visa, MasterCard or American Express).
  • Records
    If your visit is for a second opinion or on a physician's referral, you may need to bring medical records, x-rays, results from lab tests, pathology slides, or other materials. Your physician may have sent these to us already. Be sure to ask your doctor before your visit here.
  • All Medications in Original Container
    You are required to bring ALL medications to your appointment, prescription medications as well as over the counter medications.
  • Advance Directive
    An Advance Directive is a written document you prepare stating how you want medical decisions made if you lose the ability to make decisions for yourself. If you have an Advance Directive (a Living Will and/or Durable Power of Attorney for Healthcare), please bring it with you. If you do not have one, we encourage you to take the time to review and complete The Oklahoma Advance Directive for Health Care.
  • Insurance
    Just as each individual is different, so too are the vast number of health care plans. OU Physicians is contracted with a wide range of HMOs, PPOs, and other health care plans.

A driver's license or employee ID will serve as proof of ID.

Q: What if I have no insurance?
If you don’t have insurance coverage, you may receive a cash discount if all charges due that day are paid in full.  Payment arrangements may be made on an individual basis.

Patient's Bill of Rights

The Patients' Bill of Rights and Responsibilities has three goals: to strengthen consumer confidence that the health care system is fair and responsive to consumer needs; to reaffirm the importance of a strong relationship between patients and their health care providers; and to reaffirm the critical role consumers play in safeguarding their own health. The Commission articulated seven sets of rights and one set of responsibilities:

The Right to Information. Patients have the right to receive accurate, easily understood information to assist them in making informed decisions about their health plans, facilities and professionals.

The Right to Choose. Patients have the right to a choice of health care providersthat is sufficient to assure access to appropriate high-quality health care including giving women access to qualified specialists such as obstetrician- gynecologists and giving patients with serious medical conditions and chronic illnesses access to specialists.

Access to Emergency Services. Patients have the right to access emergency health services when and where the need arises. Health plans should provide payment when a patient presents himself/herself to any emergency department with acute symptoms of sufficient severity "including severe pain" that a "prudent layperson" could reasonably expect the absence of medical attention to result in placing that consumer's health in serious jeopardy, serious impairment to bodily functions, or serious dysfunction of any bodily organ or part.

Being a Full Partner in Health Care Decisions. Patients have the right to fully participate in all decisions related to their health care. Consumers who are unable to fully participate in treatment decisions have the right to be represented by parents, guardians, family members, or other conservators. Additionally, provider contracts should not contain any so-called "gag clauses" that restrict health professionals' ability to discuss and advise patients on medically necessary treatment options.

Care Without Discrimination. Patients have the right to considerate, respectful care from all members of the health care industry at all times and under all circumstances. Patients must not be discriminated against in the marketing or enrollment or in the provision of health care services, consistent with the benefits covered in their policy and/or as required by law, based on race, ethnicity, national origin, religion, sex, age, current or anticipated mental or physical disability, sexual orientation, genetic information, or source of payment.

The Right to Privacy. Patients have the right to communicate with health care providers in confidence and to have the confidentiality of their individually- identifiable health care information protected. Patients also have the right to review and copy their own medical records and request amendments to their records.

The Right to Speedy Complaint Resolution. Patients have the right to a fair and efficient process for resolving differences with their health plans, health care providers, and the institutions that serve them, including a rigorous system of internal review and an independent system of external review.
Taking on New Responsibilities. In a health care system that affords patient's rights and protections, patients must also take greater responsibility for maintaining good health.

DECLARACIÓN DE LOS DERECHOS DEL PACIENTE
La Declaración de los Derechos y Responsabilidades del Paciente tienen tres metas: fortalecer la confianza del consumidor de que
el sistema de salud es justo y sensible a las necesidades de los consumidores; de reafirmar la importancia de una relación fuerte entre los pacientes y sus proveedores de atención medica y de reafirmar el rol crítico que los consumidores desempeñan en la protección de sus propia salud. La comisión articulo sietes conjuntos de derechos y uno de responsabilidades:

El Derecho a la Información. Los pacientes tienen el derecho a recibir información precisa y fácil de entender para ayudarles a tomar decisiones informadas sobre sus planes de salud, facilidades y profesionales.

El Derecho a Elegir. Los pacientes tienen el derecho a una selección deproveedores que sea suficiente para asegurar el acceso a la apropiada atención medica de alta calidad, incluyendo proveer acceso a las mujeres a especialistas cualificados tales como los ginecólogos-obstetras y proveer a los pacientes con condiciones médicas serias y enfermedades crónicas acceso a especialistas.

Acceso a Los Servicios de Emergencia. Los pacientes tienen el derecho a acceder a los servicios de salud de emergencia cuando y donde sea necesario. Los planes de salud deben proporcionar el pago cuando un paciente acude por sí mismo a cualquier departamento de emergencia con síntomas agudos de suficiente gravedad “incluyendo dolor severo” que una “persona prudente” podría deducir que por la falta de atención medica la salud del consumidor pudiera resultar en serio peligro, deterioro grave de las funciones corporaleso disfunción seria de cualquier órgano o parte del cuerpo.

opciones de tratamiento medicamente necesarias.

Cuidado sin Discriminación. Los pacientes tienen el derecho a una atención considerada y respetuosa de todos los miembros de la industria de atención medica todo el tiempo y bajo cualquier circunstancia. Los pacientes no deben ser discriminados en la comercialización o matriculación o en la prestación de servicios de atención médica, de acuerdo con los beneficios cubiertos en su póliza y/o como lo es requerido por la ley, basado en la raza, el origen étnico, origen nacional, religión, sexo, edad, discapacidad mental o física actual
o anticipada, orientación sexual, información genética o forma de pago.

El Derecho a la Privacidad. Los pacientes tienen el derecho a comunicarse con los proveedores de atención de la salud en confianza y en tener protegidas la confidencialidad de su información médica individual identificable. Los pacientes también tienen el derecho de revisar y copiar sus registros médicos y solicitar que se los modifiquen.

El Derecho a la Resolución de Quejas Rápidamente. Los pacientes tienen
el derecho a un proceso justo y eficiente para resolver las diferencias con sus planes de salud, proveedores de salud y con las instituciones que los atienden, incluyendo un riguroso sistema de revisión interna y un sistema de revisión externa independiente.

Asumiendo Nuevas Responsabilidades. En un sistema de salud que ofrece derechos y protecciones de los pacientes, los pacientes deben asumir una mayor responsabilidad para mantener una buena salud.
